To help understand your needs, we need you to answer the questions below. We realize that most of this material may be included in your business plan or executive summary, but we'd like you to answer these questions as briefly, but as completely, as possible.

The answers to these questions are primary material used for the HPG review process. If you have an executive summary you may also send that along with your answers, but please do not answer questions by stating "See Executive Summary." Please do not provide any information in responding to these questions that you feel is highly proprietary or confidential.

The technology team will review this material in order to determine the HPG level of interest in your company and how we can provide you with value. Follow up meetings will be scheduled for those companies that demonstrate the highest likelihood of a mutually beneficial relationship with HPG.
